Symptomatic patients with unremarkable distance vision are the population this instrument exists to serve. Headache, eyestrain, intermittent blur and difficulty sustaining reading are frequently binocular in origin rather than refractive, and a distance chart offers no explanation for any of them. Fixation disparity testing at near identifies the heterophoria a patient is compensating for but not comfortably, which is precisely the finding that turns an inconclusive examination into a treatable one. In the display screen equipment population, where sustained near demand is the norm, that presentation is common enough to be routine.
Binocular vision practice uses the full complement. Foveal suppression assessment quantifies how much of one eye's central input is being discarded under binocular viewing, near point of convergence measures the limit of the convergence response, and the cover test targets address heterophoria and harmonious anomalous retinal correspondence. Together with polarised cross-cylinder balance these are the near-vision components of a proper binocular workup, presented on one illuminated card.
Presbyopic prescribing is served by the refractive content — near acuity in Snellen letters, illiterate E and Times Roman reading types, determination of the near addition, and near astigmatism testing. Amplitude of accommodation with monocular measurement under binocular conditions gives a result closer to real-world function than occluded measurement does.
Domiciliary and ward work benefits from the format. At 780 g on three AA cells with its own illumination, the unit removes the dependence on ambient room lighting that makes a printed reading card unreliable in a care home lounge or at a bedside. Two things to settle before ordering: whether polarised viewers are supplied, since four of the five panels require them, and battery life, which is unpublished.
