A Pirogoff amputation is uncommon and specific. The forefoot and midfoot are removed, the calcaneus is divided and rotated, and the remaining portion of the heel bone is fused to the cut end of the tibia. What results is a limb that ends a little below where the ankle was, retains the heel pad over its end, and can usually take weight directly through it.

That makes the level valuable for the same reason as a Syme: the user can bear weight distally, and the limb is long, which gives good control of the prosthesis. It also creates the same difficulty in a more acute form. Where a transtibial fitting has room below the socket for a foot, an adapter and a pylon, and a Syme fitting has a few centimetres, a Pirogoff fitting has less again. The socket comes down almost to the ground, and the prosthetic foot has to occupy what remains without making the limb longer than the other side.

The consequence is that a foot for this level is not a foot with a pylon beneath a socket. It is a foot with a cavity or a built-up region shaped to receive the distal end of the socket, so the two become one assembly.
