A Syme amputation removes the foot at the ankle and preserves the heel pad over the end of the tibia and fibula. What remains is a long residual limb that reaches almost to the floor, and one that can usually bear weight directly through its end, which is what makes the level valuable.

That length is also the difficulty. A transtibial amputation leaves room below the socket for a foot, an adapter, a pylon and sometimes a clamp. A Syme amputation leaves almost none. The socket descends to just above where the ankle was, and everything the prosthesis needs has to fit into the remaining few centimetres before the sole meets the ground.

This is why Syme feet exist as a distinct category rather than a size variant. A conventional foot has a substantial ankle section above the sole, designed to sit beneath a pylon. A Syme foot compresses that, putting the attachment much closer to the sole so the assembled prosthesis matches the contralateral leg length rather than exceeding it. Exceeding it is the failure mode: a foot too tall leaves the user standing with one leg longer than the other, which no amount of alignment corrects.

Attachment follows from the same constraint.
